Financial Information
Thank you for choosing Arctic Oral Surgery for your oral surgery needs. We want our patients to clearly understand their treatment needs as well as their financial responsibility before treatment begins. We therefore offer the following financial arrangements:
Patients with insurance: As a courtesy we will gladly file your claims, assist you in obtaining maximum insurance benefits and accept payment of insurance. However, the amount we collect on the day of your surgical procedure is only an estimate of your copayment. This estimate is based on information received from your insurance company. This is not a guarantee of benefit or payment. The insurance company may pay less than anticipated once the claim is received. Providing us with accurate medical and dental insurance information will help us expedite your insurance determination of benefits. Our office is more than happy to submit a pre-determination for actual coverage on your behalf.
Please remember you are responsible for all fee charged by this office regardless of your insurance coverage.
Dental benefit plans are designed to share the cost of dental care. While most plans may cover the cost of your dental services, some plans may not cover the total cost of your treatment. Dental benefit plans are not really insurance in the traditional sense but are designed to provide you with assistance in paying for your dental care. A plan may have limitations on the number of office visits, consultations, radiographs (X-rays) and various treatments it will cover.
Patients without insurance: Payment for services is due at time of treatment. We offer a 5% discount for payments made in full, (10% for patients 65 or older).

Treatment Plans: Treatment plans are estimates only. Actual fees can change if actual surgery performed changes.
Minors: If the patient is age seventeen or younger, they are a minor. A parent or guardian must accompany the patient for treatment. We realize that many families are in a state of change. Divorced, separated, single parent and blended families are now common. In many of these families the question of who is responsible for the children’s account is uncertain. The policy in our office is that the parent who requests treatment and is present for the appointment with the child is responsible for all fees incurred. Settlement must be resolved between the parents.
Short Notice Cancellation & No-Show Fee: While emergencies sometimes happen, kindly give us 24-hour notice if you must cancel or change your surgery appointment.
